Before we do, as per our security policies, we ask all visitors to the building to complete this form in as much detail as possible, this is so that we may have you on record for any future visits, as well as to go through a minor security check. ***PLEASE NOTE*** The office is closed from 16.00 on a Friday. Forms submitted after 16.00 won’t be checked by our Security Team. You are still welcome to still attend services BUT please be aware that you will need to pass a thorough security check on the door before being allowed entry. Please also bring along photo ID.
By completing this form and providing a copy of your photo ID you herby consent to the organisation making such reasonable enquiries as necessary to verify your identity prior to your proposed visit. This may include us sharing this information with CST and or the Police, for the purposes of verifying your identity. We will keep a copy of your passport for a period of 14 days after your visit, at which point it will be deleted from our system. The organisation reserves the right to decline admission whether this form is completed or not without providing reasons for doing so. We will be processing your information in line with our privacy notice, which is available on our website or from our office.
